if it is willfull, is a lawyer needed to draw out a form or can it be printed from a website. After the form is signed, what comes next?

This is the sort of question you need to ask a lawyer. What are the consequences if you don’t do it right and it doesn’t hold up in court?

You can get a durable power of attorney without a lawyer. You just fill out the form, and have it notarized. That means you can make medical decisions only. But it may vary from state to state, so it’s best to check with a lawyer as cwilbur said.
