The one child policy doesnt prevent anyone from having as many children as they like. Its just that by having more than one you give up certain benefits etc.

Also I think the rule only applies to women (since it’s not as easy to keep track of who’s the father as it is on who’s the mother). So a man could easily have several children, as long as they are with different mothers.

the one-child policy is not really in effect anymore

it is my (simplistic) understanding that if you were an only child as a result of the rule, when you grow up you are not subject to the rule. so right now only 36% of the population is still subject to that rule

the logic as it was explained to me by a 20-something chinese friend is that you can have a child to ‘replace’ each parent. i.e. a only child boy and an only child girl get married the couple can have 2 children, because those 2 ppl will need to be replaced in the work force when they can no longer work

I would think the doctors would be obligated to report it and the parents would be force to terminate the pregnacy, that would seem logical way they would do it.
