I always charge a flat rate. Never by the hour, although I calculate how many hours I think it will take to help determine the flat rate. Usage is a big part of the price. Most logo clients want unlimited usage, so I add for that.
Logo variations (seasonal, per division or department, etc.) are charged at 25% to 50% of the original price.
I always include a clause about me maintaining the right to display for portfolio purposes. This has rarely been argued.
I would suggest using the Graphic Artists Guild Pricing and Ethical Guidelines Handbook to better understand the structure of the agreement and usage rights. The suggested prices in there, however, are way too high (particularly given the economy).
